For adults. This template is written for counselling with adults. If you work with children or young people under 18, your agreement also needs to cover parental consent and responsibility, assessing a young person's understanding, and any conditions your insurer sets, so please take specific guidance from your professional body and your insurer first.

Personalise (optional)

By default this makes a general agreement you can publish on your website or print for your waiting room. No client name, no signature needed. To prepare it for one client, with their name, the date and signature lines, tick the box.

Before you add a client's name. This tool runs entirely in your browser and sends nothing anywhere, but a browser is still not a safe home for client information. Only do this on your own, password-protected device, never a shared or public one. Browser autofill and history can hold on to what you type, so clear the form when you finish. You do not need to enter anything here at all: make the general version, then write or type the client's name and the date onto the printed or PDF copy. That keeps their details out of the browser entirely, and is usually the safer choice.

About you
Suggestions update to match the body you choose. Use the exact wording of your own status.
Paste the link to your own entry on the register if you have one. Otherwise the contract links to the register search page.
Sessions
One phrase that reads on from "usually take place", e.g. "weekly and online", "fortnightly and in person at my Sheffield room", or "weekly, either online or in person, as agreed".
Fees and cancellations
Include the £ sign, e.g. £65.
Keep it fair: a charge above one session's fee can be an unenforceable unfair term under consumer law.
Supervision, records and contact
Paste a link to your privacy notice if you have one online.
e.g. 9am to 5pm, Monday to Thursday.
